It's a variation of the Jerusalem Cross, which is a Christian symbol that dates back about 1,000 years. See "Jerusalem Cross" in Wikipedia. The form has also been adopted as a variation on the traditional Celtic Cross (see photo below).

I’m pretty sure those are scalpel handles for replaceable blades. Google “Swann Morton Scalpel Handle No 3” and you wii see one quite similar to the top one in your photo.

Millsy, the crown means the knife was assayed for silver purity in Sheffield. The lion passant means it is sterling silver.
