tnjohn, that is a "folding specialized pocket instrument" (description by Miller Bro's). This link below will take you to a pic of one I posted sometime ago, the post below is Miller Bro's reply to what it is. One of the blades on yours is similar to mine, but the other is different.

They are folding surgical instruments.

Each blade had a specialized purpose.

Look at Levine's Guide, page 299.

It is called a "Bistourie", a civil-war era folding scalpel. Just sold one on eBay not too long ago.
