18th/19thC Medical Fleam

This is a small Fleam, with horn scales, by BUTLER; a device for blood letting, which was common practice for a myriad of ailments, right up into the 1950s, particularly in the farming community.
Apart from the 2 blades, the body holds a pair of tweezers and a small scalpel, or lancet, which has tortoise shell scales that fold round to become a small handle.
Due to its size, just over 2", and components, this would be for Human use and quite possibly a Military item.
